Postdated Checks (Downpayment & Monthly Amortization)

Certificate of Employment and Compensation (Notarized)
Latest Income Tax Return (ITR)
Lastest 3 mos. Payslip
Service Record (if government employee)
Copy of latest ESAV with at least 24 mos. contribution
